Caring Hospice is a unique, holistic and cost-effective service designed to meet so! cial, emotional, spiritual and physical needs of our patients and families as they face terminal illness.
Patient Care Manager is responsible for:
Coordination of all clinical services (planning, organizing, directing and supervising) provided to the hospice patient.
Overseeing the day to day clinical operation of hospice in conformance with the organization’s mission, philosophy and goals.
Ensures that services conform with all applicable laws, rules and regulations.
The Patient Care Manager is a member of the Interdisciplinary Team.
Requirements
Patient Care Manager - RN - Registered Nurse
3 years clinical experience in a home health agency or health care facility setting (hospice preferred), however will accept 2 years experience with Bachelors degree
Current, unencumbered license as a Registered Nurse in New Jersey
Bachelors of Nursing, Masters Degree preferred
Must possess a current drivers l! icense
Must meet educational/experience requirements outli! ned in the law of licensure states
Knowledge of Medicare and Medicare requirements outlined in the laws of the licensure states
Ability to relate to and treat terminally ill patients/families and their particular problems
Must possess strong patient assessment, patient management and documentation skills
Ability to function in any home situation regardless of age, race, creed, color, sex disability or financial condition of the patient
Excellent organizational, interpersonal, telephone, documentation and communication skills
Must possess a strong commitment to the goals, mission and philosophy of the organization
Ability to adapt to changing organizational needs
