Job Description
The Nursing Assistant is a para-professional health care works who has received training in basic skills and performs selected nursing care activities under the direction and supervision of a Profession! al Registered Nurse including but not limited to:
Obtain r! eports on the patient from the Charge Nurse or Primary Nurse, Introduces self to patient, reports all patient observations to the RN, takes and records vital signs (TPR) and reports any changes or altered vital signs to the RN, weighs patients as instructed recording it accurately, provides proper and safe care of patient?s clothing, assists or provides patient with bathing, grooming, dental hygiene and mouth care, personal hygiene and simple skin care and backrubs, assists patient out of bed to chair or with ambulating as instructed, change bed linens, makes occupied/unoccupied bed, provides water, nourishment and between meal feedings as instructed, test urine for sugar and acetone when ordered then reports results immediately to RN, remains with the patient at all times-leaves only when relief is provided, calls from the room via intercom to ask for relief or assistance or to report any concerns, applies anti-embolic stockings under supervision and instruction, answers p! atient and visitor questions referring all medical/nursing questions to the RN, runs errands for the patient as requested by the nurse, understands and practices medical asepsis through hand washing, care of equipment, care of linens and care of patient?s room, maintains and demonstrates Standard Precautions, understands and practices safety and fire regulations, assists and maintains a clean room, maintains patient confidentiality, security, safety through his/her conduct, is well groomed, wearing uniform and nametag, reports to and is ready to work at the scheduled time, submits completed activity sheets on a weekly basis to the agency on all patients, and performs all other appropriate CNA duties as directed by the RN.

Job Requirements
Education: Completion of at least 10 years of education; demonstrated ability to speak, understand, read and write English.
Certification: Completion of the Nurses Aide training and competency evaluation for ! a long term care facility as evident by the New Jersey Department of He! alth and Senior Services.
Health Status: Current physical examination with documentation that the aide is in good health, free of communicable diseases, and/or chronic conditions that would interfere with the performance of his/her duties.
Employability: Presenting proof of same to the Agency according to regulations set forth in the Immigration and Reform and Control Act of 1986 and related agency procedures.
