Selected candidate will have a current NJ RN license along with a minimum of 2 years' experience in an acute care envi! ronment. Case management experience, managed care experience and BSN preferred. Coordinates/facilitates patient progression through acute care hospitalization. Manages all aspects of discharge planning/transitions and performs utilization management and quality screenings. The hours of this position is 8:30 am until 4:30 pm with weekends as needed.
