More complete informations about this job opportunity kindly see the descriptions. The RN-Patient Flow functions as a caregiver for a specific patient population in order to achieve desired outcomes. He/she maintains standards of practice for nursing as defined by the ANA Code of Ethics, Nursing Scope and Standards of Practice and Nursing Social Policy Statement. This position directs and oversees the care provided by other caregivers through delegation, validation, evaluation, and coordination. This role has to coordinate the patient flow activity and assist with appropriate assessment and reassessment of patients during peak flow volume sur! ges. Assists the nurse manager to coordinate, communicate, eff! icient and effective admission/discharge/ transfer to/from the appropriate inpatient bed. This position serves as a liaison in developing a user friendly patient triage and admission system, which meets the needs of patients, physicians and staff. 1. Performs assessment/data collection in an outgoing and systematic manner, focusing on physiologic, psychological, and cognitive status.
2. Demonstrates effective nursing assessment skills as consistent with the Medical Center (HUMC) clinical guidelines.
3. In a timely manner coordinates admission, assessment, medication reconciliation form, discharges and transfers between units as needed utilizing established criteria for admission/triage of patients.
4. Responsible for ensuring patient admission profile, medication reconciliation, and discharge forms are in completion.
5. Participate in throughput activities: bed board meetings, ED presence and Ad Hoc work group to assure appropriateness of coordination! of admission/discharge/transfer from outside referral sources to the designated bed and identify and documents barriers regarding patient flow.
6. Informs appropriate department heads/administrators of patient flow activities; communicates with MDR team members and collaborates with physicians, unit medical directors, PAs and RNs.
7. Shares responsibility for the achievement of clinical, functional, fiscal and satisfaction outcomes; participates in the identification of outcome indicators and measurement tools. Administers tools to measure outcomes; tracks, trends aggregate outcome data and reports outcome analysis with recommendations.
8. Initiates and participates in the development and implementation of interdisciplinary performance improvement plans. Identifies opportunities for performance improvement in the management of designated patient populations. Identifies and investigates quality issues and sentinel events and participates in the development and i! mplementation of corrective action plans with the interdisciplinary tea! m.
9.. Orients the interdisciplinary team, to role of RN-Patient Flow triage function.
10. Maintains competence through continuing education and clinical practice and functions as a Registered Professional Nurse using the outcome based practice model by demonstrating competency in clinical judgment, select technical skills and appropriate knowledge base.
11. Assumes patient assignment when census surge in ETD.
12. Implements care in a knowledgeable, skillful, consistent and continuous manner, utilizing procedures, protocols, standards of care, and education of patients and significant others.
13. Evaluates effectiveness of care toward desired outcomes in progressing patients toward a predefined length of stay.
14. Performs documentation responsibilities following the outcome-based standards and guidelines for forms used in the Medical Center and the assigned clinical area.
15. Promotes early discharge, rapid patient through put and the ability to! meet the admission, discharge, and transfer needs.
16. Clinically knowledgeable of disease entities and changes in patients' condition and communicates with the primary care giver in a timely manner.
17. Demonstrates applied knowledge base in areas of outcome based standards, protocols, medications, standards of care, patient classification data and significant lab values as defined generically in the Medical Center and in area of clinical expertise.
18. Demonstrates the standards of behavior in accordance with the goals and mission of the medical center.
19. Promotes the medical center objectives for patient satisfaction through appropriate communication and attitudes using the AIDET as your practice guidelines.
20. Adheres to the American Nurses Association Code for Nurses with Interpretive Statements.
21. Adheres to Professional Practice Standards, using outcome based practice model and the American Nurses Association indicators as guidelines. 22. Utilizes and participates in current research findings in the care! of patients and families following standards of care, policies, and protocols.
23. Maintain current knowledge of the professional and regulatory standards and implement in your practice.
24. Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, hearing, and/or visually impaired, etc.). This process includes communicating with the patient, parent, and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.).
25. Adheres to the standards identified in the Medical Center's Organizational Competencies.

Qualifications:
Knowledge, Skills and Abilities Required: 1. Graduate of an NLN/AACN accredited program in nursing.
2. Bachelor of Science in Nursing.
3. Two to five years critical care, cardiology, intermediate care or emergency room experience required.
4. Current valid NJ State Professional Registered Nurse licen! se.
5. Maintains American Heart Association Basic Life Support HCP Certification. Knowledge, Skills and Abilities Preferred: 1. Excellent writing and communication skills.
2. Excellent assessment skills.
