Job Purpose:
The Home Health Aide is a certified paraprofessional worker, functioning in a supportive relationship to professiona! l disciplines. The Home Health Aide provides personal care on a part time basis for the sick and/ or disabled clients. The duties of a Home Health Aide are planned, assigned, and supervised by the professional nurse/therapist.
Duties:
Responsible for following all aspects of the patient plan of care as written in the patient care plan as specified by the organizational policy to include:
A) p roviding personal hygiene
B) assisting with ambulating
C) assisting with home exercise
D) assisting with medications that are ordinarily self-administered
E) providing nutritional support
F) performing other related supportive tasks
Responsible for preparing a report of activities.
Report changes in a patient's condition and needs to nursing staff.
Work with the nursing staff to provide safe comprehensive service to all agency clients helping to maximize their potential and make progress towards the goals of th! e plan of care.
Submit all paperwork with complete an! d accurate documentation according to agency time lines.
Attend the required number of in-service/continuing education hours (14) each year.
Demonstrate competency in VNA's Patient Care Manager technology.
Ability to communicate effectively with patient, their family and agency staff.
Adheres to assigned schedule and communicates any necessary changes to Home Health Aide scheduler.
Perform other duties as required.
Qualifications:
High School Diploma/ GED New Jersey Home Health Aide Certification Current Driver's License Current Auto Insurance Current Vehicle Registration
Skills:
Ability to read and write in English
Preferred Knowledge:
Knowledge of Hospice philosophy if assigned to Hospice patients
