Responsibilities:
- Implement the Re-Engineering Discharge program for EOGH.
- Develop all tools for program implementation at EOGH
- Assessment patients for discharge plan.
- Complete discharge Medication Reconciliation
- Develop Discharge Plan of Care
- Educate patient and care providers about post acute plan of care
- Conduct Post Discharge Follow up, home or skilled nursing facility
