Short Description:
As a member of an interdisciplinary healthcare team, formulates patient home care plans based upon needs asses! sments and in collaboration with the patient/family and appropriate health care team members. Acts as resource for all involved parties regarding the discharge planning process, and ensures the continuity of care in a patient family centered care model of health care delivery.
Special Requirements:
This position requires a current NJ Registered Nurse License in good standing. BSN preferred; 3-5yrs of recent clinical care experience; experience in CM/UM; interqual criteria & discharge planning experience.
