Job Function ( Overall purpose and objective of the job)
Provides direct patient care services to patients in their residence and coordinates care planning with other disciplines and the Team Leader as needed in order to achieve favorable patient outcomes through appropriate case managem! ent. No OASIS until working in field for at least 9 months. Essential Functions/Responsibilities: ( The reason this job exists is to perform these functions. These functions are performed regularly, seriously impacts other jobs, and there are a limited number of employees to whom performance of the function can be distributed).
Maintain the highest level of work ethic, and adherence to organizational policies and procedures, objectives and safety standards.
Provides direct nursing care to clients in their place of residence focusing on ensuring continuity of care and providing positive patient outcomes.
Informs the physician and other involved personnel of changes in the patient’s condition and needs.
Counsels the patient and family in meeting nursing and related needs.
Furnishes those services requiring substantial and specialized nursing skill.

Manages an assigned caseload including:

• Case management
• Chart management
• Followâ€'up visits which meet patient ! needs and conform to Agency and physician ordered frequency.
• Coordinates implementation of the plan of care in collaboration with Team Leader.
• Serves as liaison between client/physician, client/family, client and other community resources.
• Case conferences with Team Leader on prescribed schedule to ensure appropriateness of interdisciplinary delivery of care. Completes at minimum 3 joint visits per week and 1 hour of conferencing with Team Leader

Education/Certification/Experience:
A Level 1 Staff Nurse is appropriate for a new graduate or RN who has! had their license or has provided services in the Certified Home Care ! environment for less than one year and/or has less than one year nursing experience in an acute care setting. Current licensure as a Registered Nurse in the State of New Jersey. Bachelor's degree in Nursing required. One year of experience preferred but not required. Home health experience preferred but not essential. Experience cannot be substituted for education.
