The Home Care Coordinator will be responsible for:
Performing the initial assessment, annual reassessment and change of condition assessments of participants in their home environment which may include supportive housing in nursing and assisted living facilities and coordinating the plan of care with appropriate resources
Providing treatments and health education for participants and or caregivers in their home
Directing the provision of personal care, nursing rehabilitation tasks and other work of Home Health Aides and participant caregivers in the home
Acting as the liaison between the contracted home health agency and the center
Required Skills
1+ year of nursing experience in a he! althcare setting; 1+ year of working with the frail or elderly!
NJ RN License
BLS
Broad knowledge base inclusive of health care problems of the frail elderly
Knowledge of common safety hazards in the home and improvements/interventions to establish a safer environment
Knowledge of home care services, nursing principles, policies and practices with particular reference to the elderly
Competent in conducting physical assessments and developing person centered plans of care
Knowledge of health counseling/education techniques and practices
Skilled in preparing and maintaining records and written reports and responding to correspondence
Excellent organization and documentation skills; Previous management and leadership experience in Home Health Care
Familiarity with community health resources, which are available to participants and families

1+ year of experience in home care or long term care
