Hospice Nurse Practitioner (Hospice Nursing) Caring Hospice Services, a premier provider of in-home hospice care throughout the East Coast, is seeking compassionate, competent and independent Nurse Practitioners. In this challenging and immeasurably rewarding role, you will plan, organize, direct and supervise all of the clinical services provided to our hospice patients. The Hospice Nurse Practitioner will:
Provide consultation in palliative care, symptom management and supportive care to meet the needs of the patient as requested by the referring physician.
Conduct re-certifications of patients on hospice.
Consult with the referring physician, the acute care team, and other members of interdisciplinary team to contribute to the patient's plan of care.
Prepare and maintain accurate patient records, charts and documents to support the medical practice and reimbursement for services provided.

Requirements
Must be a graduate of a school accredited by the American Nurses Association
Licensed a Advance Nurse Prcatitioner in the state of NJ
Must meet the educational/experience requirement outlined in the laws of licensure States
Knowledge of Medicare and Medicaid requirements outlined in the LDC guidelines for hospice eligibility
Speaks fluent English
Access to reliable transportation with proof of liability insurance for the automobile being used to perform visits for Caring Hospice Services
