Global Health Care Group is looking for a Licensed Physical Therapist (PT) to provide services at a Skilled Nurse Facility/Long Term Care Facility, located in Wayne, NJ.

Position Overview: The PT is responsible for the assessment of referred patients that require rehabilitation services which includes; providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
Essential Duties and Responsibilities:
?Provides skilled physical therapy services/interventions in accordance with physician orders. ?Assesses patient needs, plans for, evaluates, and modifies care to meet goals of physical therapy interventions. ?Collaborates with all disciplines to plan and evaluate team goals for each patient.
Position Requirements/Qualifications License: Must have a current, full, active, and unrestricted PT license in the state of NJ. Education: Must have graduated from an accredited Physical Therapy school. Certifications: Must have current Cardiopulmonary resuscitation (CPR) certification. Professional References: Must provide reference letters or reference contact information.
