In collaboration with Hospital Operations Supervisor, facilitates patient flow by appropriately allocating resources within pre-defined protocols and systems, including but! not limited to the facilitation of departmental plans and staffing.

REPORTS TO:
Patient Flow Manager

DESCRIPTION OF WORK:
Administrative

Collaborates with appropriate leaders to facilitate departmental plans for patient admissions, discharges and transfers.

Collaborates with Staffing Resource Personnel to anticipate census 8 hours in advance on an hourly basis based on Estimated Time of Departure (ETD) in CAW, and estimates staff resource needs based on performance.

Supervises staffing and bed control staff during shift.

Ensures that sick call replacement plan from department specific plans is executed.

Maintains appropriate staff levels based on patient needs and budget.

Assesses data from units, utilizing Information Technology, to project staffing needs every 4 hours.

Makes recommendations to Hospital Operations Supervisor regarding all available staff for upcoming shi! fts.

Operational

Demonstrates proficiency in! utilization of relevant technologies including EDSB, HASB, RASB, CAW, PTS, Premise and Bed Express.

Monitors patient flow by checking HASB, EDSB, and RASB on an hourly basis to ensure all units are meeting all KPI's and allocates resources if necessary if areas are not meeting KPI's.

Monitors all status boards for census and CAP levels throughout the hospital at all times.

Oversees assignment of patients to primary and overflow/pop-up units.

Monitors Discharge CAW for ETT and ETD to ensure units are accurately predicting discharges/transfers.

Actively monitors KPIs for ETD and ETT and contacts units for performance failures.

Predicts census 4 and 8 hours ahead, on an hourly basis.

Verifies that IT Applications are completed on all units in a timely manner.

Monitors ETD in Discharge CAW and Premise to ensure prompt bed turnover.

Monitors Patient Transportation System on an hourly basis to ensur! e that transportation requests are pre-scheduled.

Reviews all room turnover performance failures with EVS Supervisor every 30 minutes

Responsible for the consistent use of patient flow designed processes and documentation of non-compliance and escalation to hospital leadership.

Communicates necessity to implement back up plans to ensure each hospital department has the necessary planned resources to care for patients according to designed processes.

Admission/Transfers/Discharges

Utilizes status boards and bed tracking system on an on-going basis to monitor admit capability and operations process compliance.

Ensure information received from units/ED is appropriate and adequate for proper bed placement.

Recommends appropriate patient placement based on pre-established criteria Med-Surg., telemetry, critical care and other specialty patient populations.

Preplans admissions using “Heads Up” pre-plannin! g for anticipated admissions.

Bed Management

Oversees! the appropriate patient bed placement; right bed first time.

Initiates High Census Protocol as designed and opens up ED Overflow and IP Pop Up areas as appropriate.

Oversees isolation patient screening status and ensures that patients removed from isolation status within 1 hour of negative screening based on pre-defined algorithms.

Resource Management

Prioritizes resources to maximize patient flow.

Assists in the planning and arranging for transfer of patients to other facilities.

Using Status Boards investigates and assists with problem-solving for delays in sending and receiving patients.

Reassesses unit status every 2 hours or more frequently and collaborates with Patient Flow Manager, Staffing Resource Manager, Hospital Operations Supervisor (if off-shift), Nurse Manager, and CSS to open unit as soon as possible when closed on HASB.

Documents all direct admit diversions in required tool.

CONTACTS/CUSTOMERS:
Frequent contacts with hospital personnel

SUPERVISORY RESPONSIBILITY:
Supervision of staffing and bed control staff during off shifts.

WORK ENVIRONMENT:
Work is active; requires frequent lifting up to 20 lbs.; occasionally lifting and carrying 20-35 lbs., and rarely lifting up to 50 lbs. Requires long periods of standing and walking, as well as occasional sitting, reaching at or above shoulder height, twisting, pushing/pulling and stooping/bending to perform patient care duties.

HAZARDS:
Potential contact with hazardous chemicals and drugs.

Use of electric machines/equipment

Potential exposure to radioactive/materials/rays.

Potential contact with combative/confused patients.

Potential exposure to biological hazards/body/body fluids/patient with infectious disease.

QUALIFICATIONS:
Degree in nursing required and demonstrated profici! ency working with patient flow/bed control activities. Ability to manag! e a changing working environment as a result of changing patient needs. Proven interaction and communication (oral, writing, presenting) skills to effectively communicate (oral and written) with management and co-workers during day-to-day, stressful, and emotionally charged situations. Demonstrated competence, clinical expertise, and sound professional judgment. Demonstrated ability to work independently and cooperatively with health care team members. Demonstrated ability to use effective analytical and critical thinking skills to problem-solve and respond quickly to meet departmental needs.

BSN in Nursing required.

Master's Degree in Nursing or related field preferred.

Minimum of three (3) years as a registered Nurse in an acute care setting required or as staff in bed control.

Previous managerial experience in nursing preferred.

Acceptable references from previous employer(s)

Able to communicate fluently (speaking and! writing) using the English Language.

OSHA REQUIREMENTS:
OSHA Blood borne Pathogen Category I

Employees in this job description are expected to have contact with blood borne pathogens and as such they are required to be oriented and annually trained under the Federal Occupational Safety and Health Administration’s Blood borne Pathogen Standard (CFR 1910.1030)
